April - Hyaline casts in urine

In the event of unusual physical exercise or fever, casts can appear in the urine. They are formed of precipitated Tamm-Horsfall protein in the distal and collecting tubules of the kidney and are washed out of the tubulus system via the papilla renalis by the flow of the urine. While detecting hyaline casts in the urine sediment is a physiological finding, casts with inclusions, such as red blood cells or drops of lipids, point towards kidney disease.
